Care Management Officer – LE4
Ace Appointments Midlands Ltd are seeking an experienced Care Management Officer for a client based in Leicester.
This is a temporary post offering hybrid working options for the right candidate.
This is a full time post that will require the successful candidate to work 37 hours per week.
The successful candidate will be required to work with older people, people with disabilities and dementia sufferer’s. You’ll work with a team of individuals who have been in the team for many years and can provide you with the support required.
As a Care Management Officer, you’ll receive referrals, carry out assessments, monitor and review cases, provide appropriate information, advice, and services to those making contact with the Department. In addition, you’ll support team duty function and hospital discharge service; receive initial contacts and referral for adult’s social care.
What you’ll need
As the ideal candidate you’ll have experienced in social care and mental health services and have the ability to carry out assessments of social cares needs. You’ll have knowledge of the resources used to support people in the community and have history working in strength-based ways. As an individual you’ll have excellent communication skills and be able to work as part of a team/partners.
The ideal candidate will hold a minimum Level 3 qualification in Health and Social care, 10 years of referable work history and be prepared to undergo an Enhanced DBS check.
